$295 for patients 16 years old and older. $195 per year for 15 years old and below.
1 Comprehensive Exam on the first year

1 Annual Exam on the first year
2 Bi-annual Exams on subsequent years
1 Emergency Exam
2 Oral Cancer Screenings
2 Fluoride Treatments
All Imaging X-Rays as needed
2 Healthy Mouth Cleanings a year (no frequency)
15% off fillings, core build ups, oral surgery, and root canals
10% off implants, crowns, veneers, bridges, partials, dentures, and night guards
$100 off on Clear Correct Orthodontic treatment

Periodontal Plan
(Active Disease Care)

$395 _{per year}

All services and offerings that are included in the Individual Plan, excluding Healthy Mouth Cleanings due to presence of infection
4 Periodontal Maintenance Cleanings

20% off Non-Surgical Periodontal Therapy (Scaling and root planning)

Family Plan
(Family Discount)

$50 off _{per person}

For immediate family members living in the same household.
For each additional family member signed up on any plan, the yearly subscription fee is discounted by $50 per person.
Maximum discount of 3 additional family members.

What else should I know?

The Membership Plan is NOT dental insurance and CANNOT be used in conjunction with ANY dental insurance plan or with ANY other discounts, including all 3rd party financing.
There are no refunds with the plan.
The plan runs for one year from the date you sign up.
Full payment of membership required.
These benefits are ONLY for the plan period of 12 months on effective date.
Membership plan cannot be utilized in hospital care settings.
Cannot be used in conjunction with dental care covered under automobile or accidental insurance coverage, or injuries covered under workmen’s compensation policies.
